THIS PROJECT INCLUDES SOURCE CAPTURE VEHICLE EXHAUST REMOVAL SYSTEM

Scope of Work – Describe the work plan that you intend to use to complete the tasks listed in the Scope of Services. Note any changes/deviations or additions to the work descriptions that may have been overlooked or that help clarify the work tasks. Deviations which demonstrate a clear benefit or advantage to the City may receive special consideration.

Scope of Services This project involves ±0.5 miles of street improvements on Willow Avenue from Shaw Avenue to Barstow Avenue. Project improvements include removing and replacing existing AC pavement, replacement of curb returns, adjustments of utility boxes, manholes, and utility valve boxes to finished grade, replacements of traffic striping, markings and signage, traffic signal modification, and reinstallation of traffic loop detectors.

Scope of Services This project involves ±0.25 miles of street rehabilitation on Bullard Avenue from Villa Avenue to Minnewawa Avenue. Project improvements include removing and replacing existing AC pavement, replacement of curb returns, drive approaches, median island cap, adjustments of utility boxes, manholes, and utility valve boxes to finished grade, replacements of traffic striping, markings and signage, traffic signal modification, and reinstallation of traffic loop detectors.

SCOPE OF WORK Contractor agrees to provide to the Department of California Highway Patrol (CHP) with all labor, materials, tools, equipment, activation fees, including travel, and every other item of expense necessary to perform Fire Alarm System Replacement at its Oakhurst Area office. Contractor agrees to provide the California Highway Patrol (CHP) Licenses: California Contractors State License Board (CSLB) C-16 Fire Protection Contractor Valid local City or County Business license Contractor shall provide a Payment Bond equal to 100 percent of the value of the Agreement. Contractor shall complete State of California Standard 807, in duplicate, and submit prior to the commencement of work.

SCOPE OF WORK Contractor agrees to provide to the Department of California Highway Patrol (CHP) Fort Bragg Resident Post, the services described herein: furnish all supplies, materials, tools, equipment, labor, personnel, and supervision; pay all taxes, insurance, bonds, license and permit fees, and all other direct and indirect costs necessary to provide Roof Replacement Services as set forth in this Agreement. License Requirement/Certification: The contractor must possess a valid C-39 California Contractor’s State License Board Specialty License. Thermoplastic Polyolefin (TPO) factory certification issued by the manufacturer. The contractor shall provide a Payment Bond equal to 100 percent of the value of the Agreement. The contractor shall complete State of California Standard 807, in duplicate, and submit prior to execution of the Agreement.

SCOPE OF WORK The project(s) described in this solicitation are critical to Facility Planning Construction and Management (FPCM) and must be completed on-time. This description establishes the minimum requirements for a complete roofing system to be delivered to California State Prison, Solano located at 2100 Peabody Road, Vacaville CA 95696. THIS IS A MATERIAL AND FABRICATION PURCHASE ONLY; THE STATE WILL INSTALL.

Scope of Work: Report shall include but not be limited to the following: date/time of service description of service provided, including size/type of replaced or repaired parts/valves and/or pipe name and signature of Contractor and/or service technician providing service Location of work Description/dimension of area work surface and any noted deficiencies and suggested corrective action Signature of Institution Contract Liaison/designee certifying indicated services were performed

Scope of Work: The Contractor shall maintain a minimum of four (4) change-out sets of air filters on site.Semi-annual and annual maintenance shall be performed during non-peak electrical months, November 1st through April 30th. With the semi-annual maintenance tasks performed during the month of November and the annual maintenance tasks being performed during the month of April.

Scope of Work: The Contractor shall prepare a dive plan prior to entering the tank. The dive plan shall be presented to the Contract Liaison or designee for review by the Institution Safety Officer, twenty-four (24) hours before entering the tank.
