The Project :The City of San Clemente is seeking proposals from qualified contractors to perform an environmental review of proposed updates to the adopted park master plan for Steed Park and provide corresponding environmental documents prepared in accordance with the California Environmental Quality Act (“CEQA”). Staff anticipates that an addendum to the IS/MND will be required. The City recently completed the conceptual design phase of a site-specific park master plan update for Steed Park. After approval and adoption, the City Council directed staff to alter the conceptual design and increase the number of planned pickleball courts.

SCOPE OF WORK A. The San Bernardino County, Department of Public Works, and the San Bernardino County Flood Control District are collectively responsible for and/or assist in the maintenance of approximately 2,500 miles of County roads and over 3,000 flood channels, basins, storm drains, dams and levees, as well as supervise 15 yards that are separated by extensive distance

SCOPE OF WORK Contractor shall provide all materials, labor, equipment, and undertake all efforts necessary or appropriate to construct the Project in accordance with the requirements of the Contract Documents, all governmental approvals, all Applicable Law, and all other applicable safety, environmental and other requirements taking into account the constraints affecting the Project Site. Except as otherwise specifically provided in this Contract, all materials, services and efforts necessary to achieve Final Completion of the Project and elements thereof on or before the deadlines provided in the Contract Documents shall be Contractor’s sole responsibility. The costs of all such materials, services and efforts are included in the Contract Sum

Scope of Services The work generally consists of header & slot grinding existing pavement, asphalt removal, placement of base pavement, placement of an asphalt level course, placement of ARHM overlay, removal and replacement of asphalt, removal and replacement of PCC curb & gutter, curb ramps, ribbon gutter, drive approaches, installation of detectable warning surface, adjustment of water valve & covers, adjustment of manhole rings & covers, installation of monument wells and other appurtenances necessary to complete the improvements as described in the Plans and Specifications.

SCOPE OF SERVICES The Work consists of the issuance of Work Orders for the reconstruction of curb and gutter, sidewalk, driveways, alley intersections, and cross gutters; and the performance of other appurtenant work at various locations within the unincorporated areas of Maintenance District 4 of Los Angeles County

Project Description The project will be constructed on the Los Angeles General Medical Center campus at 1240 N Mission Rd. Los Angeles, CA 90033, located on the corner of Zonal Avenue and Mission Road. It is the site of the former Women’s and Children’s Hospital which was demolished, and the parcel is now vacant, pending construction of the future 2-Story, 33,000 square feet Mental Health Urgent Care Center and Residential Withdrawal Management Facility. Construction of the Psychiatric Subacute Facility will be comprised of 8 Mental Health Rehabilitation Center programs that are operated separately with 16-bed each, with a total of 128 beds in a single four-story building. The total building area is approximately 122,000 square feet.

SCOPE OF WORK  Sweep clean entire roof removing dirt debris and gravel.  Apply detergent and clean with commercial power washer minimum 2,000 psi for proper adhesion.  Allow roof membrane to dry for minimum of 24 hours.  Prep deck and spot torch all vulnerable areas..  Apply one coat of primer at rate of 2.5 gallons per 100 sq feet as per material specs.  Reinforce any visual protrusion with polyester reinforcement mat.
