Cosumnes Community Services District (“District”) is seeking sealed proposals from interested artists and artist teams with experience in the design and execution of public art at the new CORE Recreation Center (“CORE”) located at 5542 Bellaterra Drive, Elk Grove, CA 95757. The artwork opportunity is for a sculpture installation integrated into an outside plaza at the front of the building.

SCOPE OF SERVICES Remove and dispose of the existing 40’ x 50’ asphalt basketball surface and base material to a depth of 7 1/2” and replace with 4” of compacted AB and 3 1/2” of class B concrete over the vapor barrier. (see Exhibit “C” Details and Specifications). Concrete shall be reinforced with 6” x 6” welded wire mesh and be pinned every 2’ to the existing concrete walkway around the perimeter. Concrete shall be saw cut to a depth of 1” in a 10’ x 10’ grid within 24 hours of pouring. Existing basketball backboard pole and footing shall be removed, disposed of, and replaced with new standard and backboard (Model # PA 666) from NEW BOUNCE (see Exhibit “C” Details and Specifications) Also, see Exhibit “C” for backboard model, colors, and rim type.

SCOPE OF WORK The Cosumnes Community Services District, in collaboration with the Cosumnes Legacy Foundation, is inviting monument builders and artist teams to submit a proposal for the Elk Grove Regional Veterans Memorial Monument Wall project. The Cosumnes Legacy Foundation has supported quality of life services for residents. The foundation’s mission is to further a legacy of community enrichment through the support of parks, recreation, fire protection and emergency medical care.

Scope of Work General Scope of Services Contractor promises and agrees to furnish to District all labor, materials, tools, equipment, services, and incidental and customary work necessary to fully and adequately supply the services set forth below and/or attached to this Exhibit A (“Services”). Service Modifications. The Party Representatives may from time to time make changes to the description of the Services; provided, however, that such changes do not increase the District’s cost by more than the amount set forth in Exhibit B. Schedule of Services. Time is of the essence in the performance of Services and the timing requirements set forth herein shall be strictly adhered to unless otherwise modified in writing in accordance with this Agreement. Contractor shall commence performance, and shall complete all required Services, no later than the dates set forth herein.

SCOPE OF WORK The District requires that the Contractor be “on-call” on a 24-hour basis for any emergency that may occur, including weekends and holidays. Response time to emergencies and routine requests is expected to be timely and proposals will be reviewed based in part on the Contractor’s ability to provide such “on-call” service and by written commitment to respond timely to both scheduled and emergency situations. The Contractor shall provide all labor, materials, tools, equipment, transportation, hauling, dumping (and fees), and incidentals necessary to perform on-call services. A clean condition is defined as the absence of residue. The Contractor shall provide a level of maintenance and performance that will present a clean, neat, and trim appearance upon completion of work. Contractor shall perform work contemplated herein in a professional manner to the satisfaction of the District.

SCOPE OF WORK a) Conduct low-frequency playground inspections on an “as needed” basis at locations specified by District staff as shown in “Attachment A”. b) Provide inspection documentation which identifies safety issues and recommended resolutions. c) Conduct follow up inspection to confirm safety issues resolved. All District playgrounds were audited at installation by Certified Playground Safety Inspectors. “Attachment A” identifies equipment locations and year of park construction. Older play equipment has been replaced. The District observes a 30-year equipment lifecycle before replacement.

SCOPE OF SERVICE: Bidder shall seal coat rubber surfacing (both PIP and Pebbleflex) for Eighteen (18) park sites. The sealant shall be an aliphatic binder. Binder must consist of Dicyclohexylmethane-4, 4’- Diisocyanate, CAS # 5124-30-1, or equivalent as approved by the District. Bidder to provide Safety Data Sheets (SDS) with bid. Bidder shall provide 6’ temporary chain link fencing around each rubber surface to be sealed to ensure no encroachment onto the surface during seal coat application and the complete drying process. Fencing cannot be placed in the fall zone of any play equipment. Fencing is to be removed within 24 hours of approved completed work.

SCOPE OF WORK BACKFLOW TESTING: Conduct annual test for approximately 541 backflow prevention assemblies managed by the Cosumnes Community Services District in Elk Grove, California. These include assemblies connected to County of Sacramento, Elk Grove Water District, and Cal American Water. Assemblies may range in size from .2” to 14” as shown below. This includes (but may not be limited to) devices from Rainbird (1 total), Wilkins (316 total), Febco (162 total), Watts (58 total), and Ames (7 total).

SCOPE OF SERVICES: The scope of services in this contract is to resurface the pool lining (currently fiberglass) with white plaster and replace specified items listed below at the Jerry Fox pool within Elk Grove Park under the outline of this scope of work and the Sacramento County Environmental Management Department (“EMD”) requirements for Pool Replastering (see Exhibit C). The pool is actually two pools separated by a wall; however, the body of water is considered one because the wall has grated equalizer openings for flow and water level balancing.
